Summer Recreation Program

Description

The Summer Recreation Program is designed to provide children with an engaging and active summer filled with a variety of activities. Participants will enjoy a mix of outdoor adventures, sports, creative arts, team-building exercises, and special theme days. Each week is carefully planned to offer a balance of structured programming and free play, ensuring children have opportunities to learn new skills, make friends, and stay active in a safe and supervised environment. The program promotes teamwork, creativity, and confidence while allowing children to explore new interests in a supportive setting.

This registration form must be submitted no later than two weeks before start of camp in order for your child/children to be accepted to the program. any forms not recieved by this date can result in your camp spot being cancelled.

PHOTO RELEASE *

• I give permission for the Happy Valley-Goose Bay (HV-GB) Summer Recreation Program to photograph my child/children during the program. I understand that this may be a photo of either my child individually or within a group. These photos may be used either within our program for program planning documentation and children's activities or may be used for promotional activities by the town of HV-GB in all media forms, including digital and print, in promotion of HV-GB Summer Recreation Program.

CONSENT FOR EMERGENCY CARE AND TRANSPORTATION *

• If, due to such circumstances as injury or sudden illness, medical treatment is necessary, I authorize the child care service provider to take whatever emergency measures s/he/them deems necessary for the protection of this child while in her/his/their care. I understand that this may involve calling a physician, interpreting and carrying out his/her/their instructions, and transporting my child to a hospital, including the possible use of an ambulance. This could also include emergency transportation required as a result of fire or other environmental emergencies. I understand that this may be done prior to contacting me, and that any expense incurred for such treatment, including ambulance fees, is my responsibility.

AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABILITY *

• In consideration of being allowed to participate in any way in the Summer recreation program, related events and Summer recreation activities, the parent/guardian acknowledges, appreciates, and agrees: 1. The risk of injury from the activities involved in this program have the possibility of being significant. 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3. I agree to comply with the stated and customary terms and conditions for participation. If, however, I observe significant hazard during participation, I will remove myself from participation.

Waiver of Liability & Assumption of Risk *

• By clicking Submit, I acknowledge that participation in the Town of Happy Valley-Goose Bay Recreation Department’s Summer Recreation Program involves activities with inherent risks, including but not limited to injuries, illness, or property loss. The Town of Happy Valley-Goose Bay, its staff, and affiliates are not responsible for any injuries, damages, or medical expenses incurred during participation. I authorize program staff to seek emergency medical care if necessary and accept full responsibility for any associated costs. I also agree to release and hold harmless the Town of Happy Valley-Goose Bay from any claims related to participation in the program. By submitting this form, I confirm that I have read, understand, and agree to all of the terms stated within.
